Community Partner Forms



Required Forms

The following forms are required to complete the service learning placement process.

A. University-Agency Agreement For Placement of Students (Form 200)
A formal agreement between CSUMB and the community-based organization. This agreement specifies expectations for each party, and formally indicates how issues of liability and worker's compensation will be handled. The contract must be renewed every 3 years. (This is a new process).

B. Organizational Profile (on-line)
Complete this form on-line to enter your organization into the Service learning Community Partners Database where student and faculty can learn about your organization’s service learning opportunities.

C. Community Site Visit Checklist (Form 300)
A tool for assessing the requirements, risks and basic safety factors of a site prior to student placement. A CSUMB staff or faculty person will visit a prospective organization and conduct a site visit. Prospective community partners of distance learners must complete and send this form to the Service Learning Institute prior to student placement.

D. Learning Agreement (Form 400)

The official document, which describes the students' specific responsibilities at their service learning site. The student, course instructor and site supervisor must sign the Learning Plan. It is kept on file by the academic department, for 3 years.

E. Student Performance Evaluation by Site Supervisor

A tool for site supervisors to provide feedback about student performance at the end of each semester.

F. Evaluation of the Service Learning Process by Community Partner
(As MS Word document) (As pdf document)
Provides an opportunity for partners to offer feedback to the SLI.

G. Service Learner Activity & Time Log
Partners verify the number of hours students serve at the placement site
